Fundraising Standing Order Form Template

NICVA has produced this form as a template for producing a standing order form for fundraising purposes.

SAMPLE STANDING ORDER FORM

  1. This form should be provided with (a) guidance on why giving in this way is so beneficial (b) the process of giving by standing order and how their information will be treated (c) the benefits and terms/conditions of Gift Aid (d) a note of thanks (e) information on where/how to return the form
  2. This form could be provided (a) in leaflet form (c) on a website to download (c) as an interactive online form
  3. The term charity is used throughout the template - however, you need not be a charity to use this fundraising method, but you will need to be a charity to claim Gift Aid
  4. The text in square brackets [  ] is for additional information and should be removed from the final form.
  5. This form is by way of example. It could be redesigned by your organisation for marketing and promotion purposes so long as it retains all the necessary information.
  6. It will be important to check that this form complies with good practice and the time of implementing it.

Gift Aid Declaration

 

I want the [Charity Name] to treat all donations that I make from the date of this declaration until I notify you otherwise as Gift Aid donations including all donations I have made for this year and the six tax years prior to the year of this declaration, and all donations I make from the date of this declaration until I notify you otherwise, as Gift Aid donations.

You must pay an amount of Income Tax and/or Capital Gains Tax at least equal to the tax that the charity reclaims on your donations in the appropriate tax year. (currently 28p for each £1 you give).
